I was so sick I could barely get off of the couch. 8 months later, I am running 10-mile races and lifting weights 6 days per week in the gym. I owe my life to Dr. Jason (JJ) Wendel in Green Hills/Nashville, TN. He listened to my symptoms/complaints with an open mind, he did exactly the procedure I asked him to perform (En Bloc/Total Capsulectomy) to spare me any further silicone or bacterial migration outside my capsules. He removed all the capsule, which is not an easy task. (Only highly skilled surgeons can get 100% of the capsule.) He performed a beautiful breast lift and a neck Platysmaplasty. I love my breasts now better than ever! Dr. Wendel is truly an artist. He is also one of the kindest, most humble and genuine people you will ever meet. That is rare to find in a surgeon. I have been told by other medical professionals that he is one of the best breast surgeons in the nation. He has performed over 10,000 explants. (Most plastic surgeons haven’t even performed 50.) I am an R.N. for 25 years. I research everything beforehand. He is hands down the best! His office staff is also some of the kindest and most knowledgeable I have met. He is worth traveling to Nashville to see.

En bloc capsulectomy refers to breast implant removal with the additional extraction of thick scar tissue, or scar capsule, that surrounds the implant(s). A capsule can grow over time, and in some cases lead to discomfort or disfiguration, causing a capsular contracture. Dr. J. J. Wendel treats capsular contracture by performing en bloc total capsulectomy, which is the safe removal of the scar capsule and breast implant together in one procedure.
En bloc capsulectomy is not a cosmetic procedure; it is meant to remove the scar capsule surrounding the breast implant(s) as well as remove any scar tissue from a ruptured implant.
There are several different breast implant revision surgeries that Dr. Wendel may suggest for you, depending on your personal circumstances. Dr. Wendel might also recommend pairing your procedure with breast implant removal, breast implant replacement, breast lift, or breast asymmetry correction to maintain the shapeliness and vitality of your breasts.
After an en bloc capsulectomy or any other breast implant revision surgery, Dr. Wendel will discuss your recovery plan with you to minimize as much discomfort as possible. En bloc capsulectomy is an outpatient procedure, so patients will go home the same day of surgery. Here are some important guidelines to follow during en bloc capsulectomy recovery:
● Ask someone to drive you home after your surgery and help you for 1-2 days.
● Rest your body and eat a balanced, nutritious diet.
● Add fresh fruit to your diet and drink plenty of liquids to alleviate constipation symptoms (due to decreased activity and pain medication).
● Take pain medication as prescribed.
● Walk as soon as possible to help reduce swelling and lower the chance of blood clots.
● Do not drive until you are no longer taking narcotic pain medications.
● Refrain from vigorous activity for 4-6 weeks.
● Return to work in 1-2 weeks.
● Resume social activities in 1-2 weeks.
● Shower 72 hours after the day of surgery.
● Avoid lifting more than 10 pounds for 2 weeks.
